Bringing health care home to you.

Lengthy hospital stays are not always practical – or desired. One of the more successful trends in health care is home health. Experts consistently point out that patients who have been ill or require rehabilitation will progress more quickly and efficiently when in their own surroundings among families and friends.

And, that’s where Carroll Home Care comes in. Whether you require professional nursing or rehabilitative care or need long-term assistance, Carroll Home Care offers an array of services in the most comfortable setting you can think of – your own home. Together with you and your physician, our interdisciplinary team designs an individualized plan of care that meets your specific needs to best meet your health care needs. In addition, the Carroll Home Care team can teach your family and other caregivers how to participate and become involved in your health care plan.

  • Pre-discharge home assessments
  • Bridge program
  • Comprehensive rehabilitation (including physical and occupational therapy and speech/language pathology)
  • Social work
  • Home health aides
  • Free evaluation and assessment
  • Nutrition assessment, education and follow up
  • Medical supplies
  • Durable medical equipment
 
Part of Carroll Hospital Center and affiliated with Carroll Hospice, Carroll Home Care is Medicare certified, licensed by the State of Maryland and accredited by the Joint Commission of Healthcare Organizations (JCAHO). In addition, Carroll Home Care is a member of the National Association for Home Care (NAHC).

Eligibility
Carroll Home Care can help determine eligibility for you, a family member or friend. You may contact us and we will call your physician to discuss your needs. Our physician’s liaison nurse also will communicate with your doctor regularly and help with the coordination of your plan of care.

You may be referred to our program through a social worker, case manager or discharge planner at your local hospital or nursing facility.
Admission guidelines
Patients who have requested admission to Carroll Home Care must meet the following criteria:
  • Are homebound
  • Have attending physician signed plan of care
  • Have a qualifying, skilled need such as nursing, speech or physical therapy
  • Reside within our service area
Medicare and other insurance coverage
Carroll Home Care is covered under Medicare Part A and Medical Assistance programs. Medicare Part B covers up to 80 percent of any medically necessary durable medical equipment.

Many major private insurance payers provide some form of home health coverage. If you belong to an HMO, coverage may be negotiated on an individual basis. Patients and.or their family members should work wtih the Intake Department and their Financial Counselor to assist with the payment negotiations.
